Aim: To verify the epidemiological profile of patients enrolled in the Mental Health Program at a Family Health Unit, as well as the most prevalent disorders, age and comorbidities. Methods: After analyzing the medical records, the following variables were obtained: diagnosis of mental health status, gender, age group, associated comorbidities and medication in use. Results: There was a prevalence of 61.3% in women and 38.7% in men;patients aged 30 to 39 years were the most affected (22.6%);the most common disorders were epilepsy (36%), depression (14%), anxiety disorder (8.6%) and schizophrenia (8.6%);the most common comorbidities were systemic arterial hypertension (30%) and insomnia (15%) and the most commonly used psychotropic drugs were amitriptyline, carbamazepine and phenobarbital (10.2% each). Conclusion: There was a high prevalence of females, mainly at 30 to 39 years old and the most prevalent diagnoses were epilepsy, depression and anxiety disorder. The operation of Mobile Mental Health Units (MMHUs) has been introdu... <strong>Background:</strong> In Greece, the provision of mental health services is limited to people residing in rural and remote areas. The operation of Mobile Mental Health Units (MMHUs) has been introduced in the Cyclades islands. It is an innovative policy intervention that has been shown to be effective and efficient internationally. <strong>Objective:</strong> The aim was to evaluate the operation of MMHUs in the Cyclades islands based on real-world evidence (RWE), from a societal perspective. <strong>Methods: </strong>A cost-effectiveness analysis was performed where outcomes and costs were elaborated and classified based on two comparators, 1) with MMHUs’ operation and 2) without MMHUs’ operation. Clinical primary outcomes were based on RWE data and were elaborated for the Disability-Adjusted Life Years (DALYs) values calculation, for a 12 months’ time horizon. Data descriptive statistics were performed with SPSS Statistics 22.0. Direct medical, non-medical and indirect costs were incorporated. Unit costs and monetary values were extracted from published data. Sensitivity analysis was undertaken to test the robustness of the results. <strong>Results:</strong> The operation of MMHUs in the Cyclades islands led to an incremental cost of €12,250.78 per DALY averted. A substantial higher increase is observed in the direct non-medical costs of the non-MMHUs’ operation where patients had to pay approximately €2,602 per capita annually for their transportation and accommodation due to hospitalization and outpatient care. Informal care expenses are 3 fold increasing in the non-MMHUs’ operation arm. Both direct non-medical and informal care costs correspond to high out of pocket payments totally covered by the patients. The sensitivity analyses demonstrated that the decision does not change when we varied the unit costs by ±10%. <strong>Conclusions:</strong> The operation of MMHUs appears to be a cost-effective option for treating patients with mental disorders in remote areas and islands in Greece. MMHUs appear to overcome the existing NHS structural inefficiencies by minimizing public expenditures and patients’ income losses by preventing and improving their mental health status. 展开更多

The Effect of Contact-Based Interventions on the Attitudes and Behaviors of Nursing Students towards People with Mental Illness: A Literature Review 认领

Background: Student nurses and other healthcare students are the future mental health professionals and have the potential for changing the future of mental healthcare. Therefore, their negative attitudes and behaviors should be screened and corrected by effective anti-stigma interventions. Otherwise, they would likely withhold some health services and practice coercive treatments once planning care to people with mental illness. However, little is known on effective approaches to correct these negative attitudes and behaviors, despite the previous reviews that have shown that contact-based interventions have demonstrated positive attitudinal and behavioral changes for nursing students towards people with mental illness. Aim: The aim of this literature review was to compile the available research evidence on contact-based interventions that have targeted the attitudes and behaviors of nursing students towards people with mental illness. Methods: The relevant literature was extracted by searching electronic databases (MEDLINE, PsycINFO, and CINAHL) and by hand checking reference lists of past similar reviews. Results: Eleven studies were retrieved and included in this review. The included studies in this review either have employed social contact interventions, video-based social contact interventions, or both types for the aim of combining or comparing. Furthermore, the included studies either have targeted the attitudes of nursing students towards people with mental illness (including prejudice), or a combination of attitudinal and behavioral outcomes (including behavioral intentions and social distance). No studies have targeted behavioral outcomes solely. Conclusions: From the available literature, the current authors cannot draw conclusions on the most effective type, form, or ingredients of contact-based interventions among nursing students, as previous literature has large variations. No consistency was found in the previous studies regarding the types or contents of effective contact-based interventions. T 展开更多

The current nosological schema for major mental disorders does not capture all the clinical phenomena, especially those being observed longitudinally (i.e., six decades of clinical observations of over eight thousand patients suffering with emotional problems of which three thousands of them suffer from major mental disorders, spanning two to three generations by the author). The objective of this article is to point out that these overlooked in their collective significance phenomena—even though they are noted by the clinicians, are not part of the categorical guidelines of the current schema. They have ramifications for better conceptualizing the road-guide in our future research for these baffling, major mental disorders, including schizophrenia, bipolar affective disorder, obsessive-compulsive disorder (OCD), anxiety, and attention deficit hyperactivity disorder (ADHD), which are sharing a common, neuro-developmental origin. 展开更多

Post-traumatic stress disorder (PTSD) is a psychiatric disorder that is the result of personal experiences directly involved with or witnessing traumatic or life-threating events. PTSD has profound psychobiological correlates, which can impair the person’s daily life and be life-threatening. The disorder is typically associated with military com-bat, acts of terrorism, sexual assault, bullying, exposure to abusive and toxic environments. The culture of prison encompasses and is the direct catalyst for consistent exposure to each of these elements. Considering that the United States leads the free world in the rate of incarceration, Prison-related PTSD is a serious public health and social issue. A sharp rise in ex-offenders that meet 3 or more of the criteria for diagnosing post-traumatic stress disorder (PTSD) from the Fifth Edition of the Diag-nostic and Statistical Manual of Mental Disorders is alarming. We review the numbers and introduce a new perspective on how the Department of Corrections and Department of Justice can play a vital role by working to correct this problem. We discuss the need to provide long-term care for this underserved population, particularly those convicted of non-violent crimes. We present arguments in support of the notion there should be professional clinical resources available for individuals released from prison to help manage the symptoms of PTSD that were created primarily through incarceration. 展开更多

Excessive distress and insomnia are much too common in the modern world and often lead to a myriad of detrimental effects including loss of cognitive ability and even physical ailments such as cancer. Current pharmaceutical treatments can be addictive, detrimental to health, and in the case of insomnia don’t produce naturalistic sleep. We present a viewpoint on a potential adjunctive treatment of distress and insomnia that harnesses specific mental imagery as a component of mind/body relaxation technique. Via our perspective on the modern nature of stress and insomnia, our theoretical perspective on how specific guided mental imagery can be used to treat these ailments, and our review on the current literature on treatment with mental imagery, we hope to stimulate further research into mental health treatment with mental imagery which has traditionally been neglected. This perspective on the pathology of insomnia and distress is founded in prevailing “dysevolution” and hyper-arousal theories. Hyper-arousal is characterized in part by a vicious cycle of chronic physiological and emotional stimulation/distress. We argue for spatially based mental imagery in the form of nighttime-sky imagery to attenuate such pathology by breaking one away from a vicious cycle of stimulation and distress and discuss neuropsychological bases for its potential treatment mechanisms which include the autonomic nervous system and a phenomenal foundation of conscious cognition. 展开更多

Analyzing Drivers’ Mental Patterns Using Q-Methodology 认领

The human factor is the most important cause of road accidents. Investigating the drivers’ mental patterns can lead to a better understanding of the factors that affect drivers to make a mistake and thus increase the likelihood of an accident. In this study, mental patterns of drivers as a human characteristic are determined through a questionnaire survey. To do this, 166 participants (18 - 65 years) were asked to express their opinion on the possible effect of 25 factors on the occurrence of accidents. These factors were selected through the investigation of the accident database during the last three years in different areas of the case study. To analyze the data extracted from the survey, Q-methodology was used. The results of the factor analysis showed that there are 5 mental patterns among the participants. Based on the driver’s opinion, human factors and road conditions were the most and least influential accident-generating items, respectively. The most significant reason for accidents determined by drivers was human errors including 1) unauthorized overtaking, 2) unauthorized speed, 3) driver distractions (such as cell phone), and 4) driver physical disability (such as visual impairment). Moreover, the failure of the vehicle was mostly reported as another influential contributor to accidents. It is worth mentioning that the results of this study can be used to minimize accidents resulted from drivers’ behavioral errors by suggesting strategies for enhancing their performance through new manuals which is a step towards a safer road. 展开更多

Mental Wellbeing Monitoring in a Sample of Emergency Medical Service Personnel 认领

Background: There have been concerns regarding the mental wellbeing of emergency medical service staff, particularly when faced with multiple trauma exposures. This study aimed to describe a cross-sectional view of the mental wellbeing status of emergency medical service workers, as well as to comment on the usefulness of an electronic survey tool to reach staff that is distributed across large geographical areas. Methods: The data presented here are drawn from an anonymous, voluntary, electronic survey, made available via the internet, to emergency medical service workers in the Western Cape province of South Africa. This paper reports on four domain expressions of mental wellbeing, namely disordered mood, problematic substance use, post-traumatic stress disorder, and domestic discord. Results: Elevated rates were reported by this sample, across all four distress domains, i.e. mood disorder = 12.3%;problematic substance use = 14.9%;post-traumatic stress disorder = 11.9%;and domestic discord = 11.9%. These rates were generally similar to data from comparable emergency worker groups, and much higher than local population estimates. Conclusions: The data support earlier findings that emergency medical work is associated with increased risk for symptoms of psychological distress, which present a strong case for regular organizational monitoring of employees’ mental wellbeing. In this regard, a brief, internet delivered, survey-type tool showed promise for screening employees, in order to guide further streaming of distressed individuals towards appropriate support services. Further work is required to develop clear mechanisms for referral and intervention in order to optimize a confidential and supportive monitoring program. 展开更多

Post-stroke depression (PSD) is one of the common complications of stroke. PSD has been a major factor hindering the recovery of neurological functions and daily activities in stroke patients and is closely related to the social avoidance and increased mortality of stroke patients. Although antidepressant drug treatment has improved during the last decades, symptoms in about 20% of the patients are not in remission two years after initiation of pharmacological intervention. Nowadays, non-invasive brain stimulation;techniques;transcranial direct current stimulation (tCDS) has gained an important interest in the treatment of many neuropsychiatric disorders which might be of some help in treatment of PSD. The goal of the current study is to evaluate the beneficial short term role of tDCS on post-stroke depression. Study Design: This was a randomized double blind placebo-controlled clinical trial included forty hemiparetic patients with post-stroke depression from outpatient clinic of Faculty of Physical Therapy, Cairo University, in the period between June 2018 to June 2019. All patients diagnosed as having a PSD according to the Diagnostic and Statistical Manual of Mental Disorders (DSM-5). Depression was evaluated by Hamilton Depression Rating Scale (HAM-D) pre and post treatment and quality of life was evaluated by the Stroke Specific Quality Of Life scale (SS-QOL). Patients were assigned randomly into two groups: the study group A and the control group B. Both groups received antidepressant medication in addition to physical therapy program. Group A received tCDS for 30 minutes, three times per week for one month, while the group B received sham transcranial direct current stimulation. Purpose: A literature review was conducted to explore the social factors and mental health-related scales, and to examine associations between social factors and mental health of the elderly, in order to consider the social aspects and mental health as outcome measurements for an effective community-based care. Methods: An electronic search for English articles published in peer-reviewed journals up to October 2019 was performed using PubMed, MEDLINE, and CINAHL with the following search terms: “social cohesion,” “mental health,” “elderly,” and “community.” Inclusion criteria were 3 as follows;1) studies in community-dwelling elderly people or participants including elderly individuals;2) studies with clear descriptions of social factors in the Methods section;and 3) studies with clear descriptions of health-related items (e.g., mental health) in the Methods section. The extracted articles were subjected to narrative review. Results: In total, 118 articles were identified, of which 11 met the inclusion criteria after critical review of the full text. 6 mental health-related scales were perceived in order to assess of mental health or depression or anxiety of the elderly. And, 4 factors related to social aspects;neighborhood social cohesion, social capital, neighborhood environment, and social support, were perceived. Significant associations between social aspects and mental health of the elderly were examined. In social cohesion, 6 studies reported a significant association with elderly mental health. In social capital, 3 studies reported a significant association with elderly mental health. In neighborhood environment, 3 studies reported a significant association with elderly mental health. Discussion: In the development of an effective community-based care, it may be expected to be focused on social aspects to lead to promoting mental health of the elderly. The aim w... <strong>Background:</strong> Information and communication technologies have revolutionized education, due to development of didactic strategies to take advantage of them for maximizing learning. The aim was to analyze the influence of immersion tools as a strategy to improve adolescents’ mental health. <strong>Methods:</strong> Qualitative research was employed, and participants’ experiences, opinions, values, behaviors, emotions, feelings were recorded. Twenty six students of both sexes, aged 13 to 15 years, from the three grades of secondary school were selected by convenience, and supported by the school’s psychologist. A workshop on emotional intelligence (EI) based on educational technology was implemented using a video;students practiced a reflexive process to understand themselves and their surroundings, creating a video + audio as a product of the workshop. In it, students analyzed several problems they face and their relationship with their contextual situation. Data collection was through semi-structured interviews before and after the workshop on EI. Data analysis was according to Souza Minayo, and the theoretical framework according to Goleman, Tapia <em>et al.</em>, Gorostieta <em>et al.</em>, and Vigotsky. <strong>Results: </strong>Two categories emerged: Category <strong>I</strong>, EI as a risk factors transformer from living with addictions to protection factors in adolescents. Category <strong>II</strong>, EI development through immersion tools to improve mental health in adolescents. <strong>Conclusions: </strong>Immersion tools favor user to be closer, with a humanist focus where real necessities are studied after the vivid experience of the problem to solve or get a better situation. The goal is that students experience real situations inside controlled surroundings, in such a way that they learn from their experiences and acquire knowledge through self-learning. EI program through immersion tools favored to modulate feelings, and to get a better psychological and social adaptation. To identify self-emotions and others’ with higher precision in different situations allowed reflexive and clear thoughts on the contextual events, improving mental health. Nursing has an important role in prevention and health promotion programs, its participation in the health team to care for mental health, mainly in vulnerable groups, is very relevant. 展开更多

Some of the major issues faced by the adolescents include: mental health problems, early pregnancy and childbirth, (HIV/STI) and other infectious diseases, violence, injuries and malnutrition. The present study aims to understand the effectiveness of peer-led intervention on knowledge, attitude and practices related to sexual reproductive and mental health issues in adolescents among participants and non-participants. The study was carried out to improve the health-seeking behaviour of adolescents on sexual reproductive and mental health issues in low resource setting across four states in India. <strong>Methods:</strong> The study adopted a cross-sectional survey which was conducted on 400 adolescents from each of the four districts namely Bangalore, Khagaria-Bihar, Khed-Pune and Sahinganj-Jharkhand. A comparative analysis was conducted between the participants and the non-participants of the intervention to understand the effectiveness of the intervention on the KAP related to sexual reproductive and mental health of adolescents.<strong> Results:</strong> The study shows that the participants from Karnataka and Maharashtra had better knowledge in emergency contraception, HIV awareness, use of contraceptives and consumption of IFA’s than the non-participants;both the participants and the non-participants showed a positive behaviour in help-seeking in case of psychological challenges. <strong>Conclusion:</strong> The peer-led approach was found to be an effective strategy for learning, sharing and monitoring adolescent sexual reproductive and mental health. 展开更多

A classroom-based parent interview was designed and implemented in an undergraduate psychiatric mental health nursing class to fill the gap between nursing students and parents of child or adolescent patients with mental health issues faced during clinical. The goals of this learning activity were to increase understanding of what parents experience when dealing with their child’s mental health problems and to increase student engagement and attention. The class using this learning activity consisted of three parts: 1) an assigned pre-class reading;2) a mini-lecture;and 3) a parent interview presentation. Students were pre-assigned a reading chapter and faculty-developed interview questions. During the mini-lecture, important knowledge related to mental health care of children was assessed using CourseKey software. After the mini-lecture, the parent guest speaker delivered her presentation about herself, her child’s strengths and abilities, her child’s mental health problems and their impact, and her family’s strengths based on the interview question prompts for about 30 minutes, followed by 20 minutes of a Q & A session. The set of interview questions was developed by faculty based on the competency questions of the Child Behavior Check List. Despite some limitations, this classroom-based parent interview using a flipped classroom model was found to be a meaningful learning strategy by increasing student engagement and attention, increasing retention of knowledge learned in class, and filling that gap in clinical. 展开更多

The aim of this paper was to describe the routine of Brazilian nursing students, identifying the activities that are part of their daily life, highlighting how much time they dedicate for physical activity practice in their leisure time. The participants of this cross-sectional study were 141 nursing students from a public university in the State of S<span style="white-space:nowrap;">&#227;</span>o Paulo. Data were collected using a sociodemographic questionnaire, three closed questions about physical activities and one open question in which the students described their daily life activities. For analysis, we used the chi-squared-test and content analysis. Although academic, household and work activities take up most of the students’ week, some dedicate a portion of their daily live to enjoy leisure periods with physical activities, sedentary behavior and sociocultural activities. Some options of activities seem to be related to Brazilian cultural aspects that are pointed out in this paper. Some recommendations were made, aiming at improving the health of students as well as the importance of a healthier school environment. 展开更多

Women diagnosed with breast cancer may have serious psychological problems and will suffer from adjustment disorder(AjD).We investigated the prevalence of AjD in female breast cancer patients who were diagnosed within 1 year and examined the severe life events they experienced,and the most common symptoms of AjD.342 newly diagnosed(<1 year)female breast cancer patients were recruited from Tongji Hospital and Hubei Cancer Hospital in Hubei,China,from July 2018 to May 2019.The patients completed the self-report questionnaire including demographic characteristics and the scale ADNM-20 for the diagnosis of AjD.SPSS20.0 was used for data analysis.As a result,the prevalence of AjD in breast cancer patients was 38.6%.Patients from rural areas and lacking of exercise were more likely to suffer from AjD(P<0.05).The prevalence of AjD in patients who did not regard breast cancer as the most severe life event was higher than that in patients who took breast cancer as the most severe life event(44.4%vs.33.9%,P<0.05;OR=1.728,95%CI=1.072-2.787).The symptom that scored highest was preoccupation(3.15).We found that the prevalence of AjD in women with breast cancer in this study is very high and warrants more attention.Patients from rural areas,lacking of exercise and subject to multiple stressors are more likely to suffer from AjD.The commonest and severe symptom is preoccupation. 展开更多

Objective:Based on the health belief model,the depression status of medical students was analyzed in order to provide new ideas for the intervention of mental health problems of medical students.Methods:By using the method of stratified cluster sampling,1053 medical students in a medical college in Hainan province were investigated with self-designed general situation questionnaire,SCL-90,CSQ and emotional management health belief questionnaire.Results:The positive rate of depressive symptoms in medical students was 17.5%.Multivariate unconditional Logistic regression analysis showed that gender,coping style,"perceived obstacles","perceived severity","perceived susceptibility"and"self-efficacy"were related factors of depression in medical studengts.Conclusion:The related factors of depression in medical students include personal factors and the health belief factors such as"perceived obstacles","perceived severity","perceived susceptibility"and"self-efficacy".Psychological intervention based on Health belief model can be used for intervention of medical students. 展开更多

Nutrition plays a key role in brain development,mental health,and psychiatric disorders.The role of omega-3 polyunsaturated fatty acids in physical health is well established,and their role in mental health is becoming increasingly evident.Omega-3 fatty acids are involved in a wide range of physiological functions that are related to neurogenesis,neurotransmission,and neuroinflammation;therefore,they play fundamental roles in the development,functioning,and aging of the brain.In humans,dietary deficiencies of omega-3 fatty acids are associated with an increased risk of developing various psychiatric disorders,including depression,bipolar disorder,schizophrenia,dementia,attention-deficit/hyperactivity disorder,and autism.In particular,eicosapentaenoic and docosahexaenoic acid have been linked to the maintenance of mental health,and their deficits have been implicated in the pathophysiology of mental disorders.This may be mediated by the modulation of inflammatory processes and their direct effects on neuronal membrane fluidity and receptor function.However,randomized clinical trials that have investigated the therapeutic effects of omega-3 fatty acids have yielded inconclusive results,thereby limiting the use of these nutrients in psychiatric practice.High-quality clinical trials should be conducted to examine the efficacy of omega-3 fatty acids in preventing and treating mental disorders.The undesirable side effects of omega-3 fatty acid supplementation should also be considered.These effects may become apparent after many years of administration,and therefore,they may not be detected in most cases. 展开更多
