Objective Patients with repaired tetralogy of Fallot(rTOF)account for the majority of cases with late onset right ventricle(RV)failure.The current surgical approach,including pulmonary valve replacement/insertion(PVR),has yielded mixed results with some patients recover RV function and some do not.An innovative surgical approach was proposed to help ventricle to contract and improve RV function qualified by ejection fraction with one or more active contracting bands.Computational biomechanical modelling is a widely used method in cardiovascular study for investigation of mechanisms governing disease development,quantitative diagnostic and treatment strategies and improving surgical designs for better outcome.Muscle active contraction caused by zero-load sarcomere shortening leads to change of zero-load configurations.In lieu of experimenting using real surgery on animal or human,computational simulations(virtual surgery)were performed to test different band combination and insertion options to identify optimal surgery design and band insertion plan.Methods Cardiac magnetic resonance(CMR)data were obtained from one rTOF patient(sex:male,age:22.5 y)before pulmonary valve replacement surgery.The patient was suffering from RV dilation and dysfunction with RV end-systole volume 254.49ml and end-diastole volume 406.91 mL.A total of 15 computational RV/LV/Patch/Band combination models based on(CMR)imaging were constructed to investigate the influence of different band insertion surgery plans.These models included 5 different band insertion models combined and 3 different band contraction ratio(10%,15%and 20%band zero-stress length reduction).These models included 5 different band insertion models:Model 1 with one band at anterior to the middle of papillary muscle;Model 2 with one band at posterior to the middle of papillary muscle;Model 3 with 2 bands which are the ones from Models 1&2 combined;Model 4 with a band at the base of the papillary muscle;Model 5 with 3 bands which is a combination of Models 3&4.A pre-shrink 展开更多

Lack of conclusive beneficial effects of strict glycemic control on macrovascular complications has been very frustrating for clinicians involved in care of patients with diabetes mellitus (DM). Highly publicized controversy surrounding cardiovascular (CV) safety of rosiglitazone resulted in major changes in United States Food and Drug Administration policy in 2008 regarding approval process of new antidiabetic medications, which has resulted in revolutionary data from several large CV outcome trials over the last few years. All drugs in glucagon-like peptide-1 receptor agonist (GLP-1 RA) and sodium-glucose cotransporter-2 (SGLT-2) inhibitor classes have shown to be CV safe with heterogeneous results on CV efficacy. Given twofold higher CV disease mortality in patients with DM than without DM, GLP-1 RAs and SGLT-2-inhibitors are important additions to clinician’s armamentarium and should be second line-therapy particularly in patients with T2DM and established atherosclerotic CV disease or high risks for CV disease. Abundance of data and heterogeneity in CV outcome trials results can make it difficult for clinicians, particularly primary care physicians, to stay updated with all the recent evidence. The scope of this comprehensive review will focus on all major CV outcome studies evaluating CV safety and efficacy of GLP-1 RAs and SGLT-2 inhibitors. 展开更多

OBJECTIVE:To examine the clinical efficacy of Jiawei Shenfu decoction on tumor necrosis factor-alpha (TNF-cα) and nuclear factor-kappa B (NF-KB) levels in patients who have chronic heart failure with syndromes of deficiency of heart Yang.METHODS:A total of 63 patients with syndromes of deficiency of heart Yang (chronic heart failure)were enrolled.Patients were randomly divided into the control group and Jiawei Shenfu group.All patients received standard medications for treatment of chronic heart failure.Patients in the Jiawei Shenfu group were additionally provided Jiawei Shenfu decoction one dose daily.Treatments continued for 4 consecutive weeks.The primary endpoint was the change in plasma B-type natriuretic peptide (BNP),NF-KB,and TNF-cα levels during 4 weeks of treatment.RESULTS:At the 4-week follow-up,a significant reduction in BNP levels compared with baseline was observed in both groups,but the Jiawei Shenfu decoction group showed a significantly greater reduction than did the control group.The Jiawei Shenfu group also showed superior performance regarding the Minnesota Living with Heart Failure Questionnaire score,the Chinese medicine syndrome score,heart rate,left ventricular ejection fraction,and 6-min walking distance compared with the control group.The degree of changes in NF-KB and TNF-α levels in the Jiawei Shenfu group was more significant than that in the control group.CONCLUSION:Routine medicine combined with Jiawei Shenfu decoction for patients with heart Yang deficiency syndrome in chronic heart failure can improve the left ventricular ejection fraction and cardiac function,and reduce BNP levels.The mechanism may be related to inhibition of pro-inflammatory cytokines and the NF-KB-induced kinase pathway,leading to amelioration of the inflammatory response. 展开更多

目的:探讨心脏超声在重症监护室(ICU)肺动脉高压(PAH)患者肺血管压力及右心功能评估中的价值。方法:选择ICU内疑似存在右心功能障碍31例并发PAH(观察组)患者和35例非PAH(对照组)患者作为研究对象。患者均采用便携式彩色多普勒超声诊断... 目的:探讨心脏超声在重症监护室(ICU)肺动脉高压(PAH)患者肺血管压力及右心功能评估中的价值。方法:选择ICU内疑似存在右心功能障碍31例并发PAH(观察组)患者和35例非PAH(对照组)患者作为研究对象。患者均采用便携式彩色多普勒超声诊断仪对胸骨旁大动脉轴切面和心尖四腔心切面做重点观察。对比两组肺动脉主干直径、右肺动脉直径、肺动脉收缩压(PAPS)、肺动脉舒张压(RADP)、肺动脉平均值(PAPm)、右心室射血分数(RVEF)、右心室面积变化率(RVFAC)、三尖瓣环收缩期移位(TAPSE)、右心室心肌功能Tei指数(RVTei)及E/A心脏超声检测结果。结果:方差分析显示,对照组与观察组各亚组患者肺动脉主干直径、右肺动脉直径、PAPs、RADP、PAPm、RVEF、RVFAC、RVTei指数及E/A值间比较,差异均存在统计学意义(P<0.05)。SKN-q检验显示,观察组轻度亚组患者肺动脉主干直径、右肺动脉直径、PAPs、RADP、PAPm、RVTei指数及E/A值大于对照组,中度亚组显著大于轻度亚组,重度亚组显著高于中度亚组(P<0.05);而观察组RVEF、RVFAC超声诊断值小于对照组,中度亚组显著小于轻度亚组,重度亚组显著小于中度亚组(P<0.05)。独立样本t检验显示,观察组总体肺动脉主干直径、右肺动脉直径、PAPs、RADP、PAPm、RVTei指数及E/A值均显著高于对照组,RVEF、RVFAC显著低于对照组(P<0.05)。而各组TAPSE超声测定值间比较,差异无统计学意义(P>0.05)。结论:便携式心脏超声检查可实时客观的反应PAH患者肺血管压及右心功能状态,为临床PAH的临床筛查及治疗提供有效信息。 展开更多

目的探讨健康人群成纤维细胞生长因子23水平与衰老相关心脏舒张功能亚临床状态改变的相关性及二者相关性的性别差异。方法选取国家973衰老课题组2014年1月至2015年2月中国医科大学研究人群716例,按性别、年龄分为4组:<60岁男性组、... 目的探讨健康人群成纤维细胞生长因子23水平与衰老相关心脏舒张功能亚临床状态改变的相关性及二者相关性的性别差异。方法选取国家973衰老课题组2014年1月至2015年2月中国医科大学研究人群716例,按性别、年龄分为4组:<60岁男性组、女性组,≥ 60岁男性组、女性组。检测受试者一般临床资料及血液生化指标,以亚洲人群改良的慢性肾脏疾病流行病学协作组(CKD-EPI-ASIA)公式估算肾小球滤过率(eGFR CKD-EPI-ASIA)。测量并记录受试者超声心动图心脏结构及舒张功能指标:左房内径(LAD)、室间隔厚度(IVST)、左室后壁厚度(LVPWT)、左室质量指数(LVMI)、左房质量指数(LAVI)及舒张早期二尖瓣血流峰值速度E与舒张早期二尖瓣瓣环峰值速度e′的比值(E/e′)。应用酶联免疫吸附试验双抗体夹心法测定成纤维细胞生长因子23(FGF-23)、C反应蛋白(CRP)及白细胞介素6(IL-6)。以二元Logistic回归方程按三分位数分层法分析FGF-23水平与衰老相关心脏舒张功能亚临床状态的相关性及二者相关性的性别差异。结果(1)血清LogFGF-23水平男性与女性相比,差异具有统计学意义[(2.0±0.3)ng/L比(1.9±0.4)ng/L,P<0.05]。(2)心脏舒张功能随年龄增长而逐渐减低,且女性舒张功能随年龄下降较男性显著[E/e′:<60岁男性组(7.6±2.6),≥60岁男性组(8.6±2.7),P<0.01;<60岁女性组(8.3±2.3),≥60岁女性组(9.5±3.1),P<0.01;LAVI:<60岁女性组(14±5)ml/m2,≥ 60岁女性组(16±5)ml/m^2,P<0.01]。(3)FGF-23与总体人群中年龄、LAVI存在相关性。FGF-23与男性E/e′呈显著正相关(r=0.149,P<0.05),而女性无显著相关性。(4)二元Logistic校正混杂变量后显示,男性中位数水平及高位数水平FGF-23与心脏舒张功能增龄性下降独立相关(OR=2.831,95%CI 1.144~7.009,P=0.024;OR=2.548,95%CI 1.053~6.163,P=0.038)。结论健康人群血清FGF-23水平与衰老相关心脏舒张功能亚临床改变有关,高水平FGF-23与男性� 展开更多

目的探讨模拟月尘(lunar dust simulant,LDS)对大鼠心率变异性(heart rate variability,HRV)及系统炎症损伤的影响。方法24只大鼠随机分为对照组(Con),LDS低(0.35mg/只大鼠)、中(1.05mg/只大鼠)、高(3.15mg/只大鼠)3个剂量组,通过气管... 目的探讨模拟月尘(lunar dust simulant,LDS)对大鼠心率变异性(heart rate variability,HRV)及系统炎症损伤的影响。方法24只大鼠随机分为对照组(Con),LDS低(0.35mg/只大鼠)、中(1.05mg/只大鼠)、高(3.15mg/只大鼠)3个剂量组,通过气管滴注染毒法建立模拟月尘损伤模型。记录大鼠心率和心率变异性(heart rate variability,HRV)变化,并观察大鼠心脏结构病理变化和血清中炎症因子变化。利用qPCR和Western blot法检测大鼠心脏组织中内皮型一氧化氮合酶(endothelial nitric oxide synthase,eNOS)、诱导型一氧化氮合酶(inducible nitric oxide synthase,iNOS)和细胞间黏附分子-1(intercellular cell adhesion molecule-1,ICAM-1)基因和蛋白表达水平。结果与对照组相比,在中、高剂量暴露组中心率明显降低,HRV明显改变(P<0.05,P<0.01)。HE染色显示心脏病理特征随LDS剂量的增加而增强,血清检测结果表明系统炎症因子水平增加(P<0.05,P<0.01)。LDS3个剂量暴露组心脏组织iNOS表达明显上调,中、高剂量组ICAM-1表达明显上调(P<0.05,P<0.01)。结论 LDS暴露导致系统炎性病变,影响自主神经功能。同时,LDS可激活血管内皮细胞,损害内皮功能,具有导致心血管疾病的潜在风险。 展开更多

Objective: To investigate the effect and safety of Guanxinning Tablet(冠心宁片, GXN) for the treatment of stable angina pectoris patients with Xin(Heart)-blood stagnation syndrome(XBSS). Methods: One hundred and sixty stable angina pectoris patients with XBSS were randomly assigned to receive GXN(80 cases) or placebo(80 cases, Guanxinning simulation tablets, mainly composed of lactose), 4 tablets(0.38 g/tablet), thrice daily for 12 weeks. After treatment, an exercise stress test(treadmill protocol), Chinese medicine(CM) syndrome score, electrocardiogram(ECG), and nitroglycerin withdrawal rate were evaluated and compared in the patients between the two groups. Meanwhile, adverse events(AEs) were evaluated during the whole clinical trial. Results: Compared with the control group, the time extension of exercise duration in the GXN group increased 29.28±17.67 s after treatment(P>0.05);moreover, the change of exercise duration in the GXN group increased 63.10±96.96 s in subgroup analysis(P<0.05). The effective rates of angina pectoris, CM syndrome and ECG as well as nitroglycerin withdrawal rate were 81.33%, 90.67%, 45.76%, and 70.73%, respectively in the GXN group, which were all significantly higher than those in the control group(40.58%, 75.36%, 26.92%, 28.21%, respectively, P<0.05). Conclusion: GXN was a safe and effective treatment for stable angina pectoris patients with XBSS at a dose of 4 tablets, thrice daily. 展开更多

Conventional recommendations for diagnostic testing for the evaluation of stable ischemic heart disease in women have largely paralleled those in men.Although they are designed primarily for the identifi cation of obstructive coronary artery disease(CAD),traditional approaches can lead to overtesting in women without differentiating who is truly at risk.Several unique factors related to the presentation,diagnosis,and underlying pathophysiology of stable ischemic heart disease in women necessitate a more specific approach to the assessment of their risk,complete with separate guidelines when appropriate.This overview highlights how advanced noninvasive imaging tools,including cardiac computed tomography angiography,positron emission tomography,and cardiac magnetic resonance imaging,are enabling very sensitive assessments of anatomic atherosclerotic plaque burden,macrovessel-and microvessel-related ischemia,and myocardial fi brosis,respectively.Moving forward,effective diagnostic testing will need to identify women at high risk of adverse cardiovascular events(not anatomically obstructive CAD per se)without overtesting those at low risk.Judicious application of novel imaging approaches will be critical to broadening the defi nitions of CAD and ischemia to better refl ect the whole spectrum of pathological phenotypes in women,including nonobstructive CAD and coronary microvascular dysfunction,and aid in the development of needed evidence-based strategies for their management. 展开更多

Objective: To investigate the effect of Rhodiola sachalinensis combined with trimetazidine on patients with coronary heart disease complicated with heart failure and the influence of RDW (Red Blood Cell Distribution Width), GDF-15 (Growth and Differentiation Factor 15) level. Methods: A total of 104 patients with coronary heart disease and heart failure treated in our hospital from March 2016 to November 2018 were randomly divided into control group and observation group, 52 cases in each group. After admission, patients in the control group were given routine symptomatic treatment according to the course of the disease, including angiotensin-converting enzyme inhibitors or angiotensin receptor antagonists, diuretics, beta-blockers and other drugs. The observation group was treated with Dazhu Hongjingtian injection combined with trimetazidine on the basis of the treatment measures of the control group. At the end of the course of treatment, the therapeutic effects of the two groups were evaluated, and the indexes of cardiac function before and after treatment were tested, including:left ventricular end-diastolic diameter (LEVDD), left ventricular end-systolic diameter (LVESD), left ventricular ejection fraction (LVEF, Left Ventricular End-Diastolic Dimension), left ventricular ejection fraction (Left Ventricular Enje). The changes of RDW and GDF-15 levels before and after treatment were analyzed and compared between the two groups. Results: The total effective rate of the observation group was 95.00%, which was significantly higher than that of the control group. After treatment, the LVEF value of the observation group was significantly higher than that of the control group, but the LVESD and LVEDD of the observation group were significantly lower than that of the control group. After treatment, the content of GDF-15 in the observation group was significantly lower than that of the control group;the RDW of the observation group was lower than that of the There was no significant difference. Stress is a potent risk factor for depression.Chronic stress can exacerbate and induce symptoms of depression.Clinical studies suggested that depressive patients are more likely to develop coronary artery diseases.However,the causal relationship between depression and heart failure progression remains unclear.In this study,we aimed to explore the relevance between stress and heart failure(HF)in a mouse model subjected to chronic restraint stress and left anterior descending coronary artery(LAD)ligation.Mice were restrained for 3 h daily for 21 days and the processes were repeated once 3 months later.After the repeated chronic restraint stress,mice showed dramatically increased immobility time in the forced swim test,indicating a state of despair.Restrained and control mice were further subjected to LAD ligation surgery.Echocardiography was conducted 1 week,2 weeks,and 1 month afterward.LAD-operated mice showed a significant decrease in the values of left ventricular ejection fraction(LVEF),and there was no difference in the LVEF values between the restrained and control mice.Relevant gene expression,neurotransmitter system,glial activation,and morphology of the heart–brain axis were comprehensively evaluated.We found no overall differences between the restrained and control mice with HF.Our results revealed that the repeated chronic restraint stress may have little effects on the progression of heart failure. 展开更多

目的:探讨藏紫菀总黄酮对高原缺氧所致小鼠心肌损伤的保护作用及作用机制。方法:实验分为正常对照组、缺氧模型组、芦丁组和藏紫菀总黄酮组,除正常对照组外,其余3组小鼠在低压低氧动物实验舱内减压缺氧12h,对各组小鼠心肌组织进行病理... 目的:探讨藏紫菀总黄酮对高原缺氧所致小鼠心肌损伤的保护作用及作用机制。方法:实验分为正常对照组、缺氧模型组、芦丁组和藏紫菀总黄酮组,除正常对照组外,其余3组小鼠在低压低氧动物实验舱内减压缺氧12h,对各组小鼠心肌组织进行病理显微结构观察,测定小鼠心肌组织中的MDA、H2O2含量及LDH、CK活力,Western blot检测HIF-1α和VEGF蛋白的表达水平。结果:缺氧能够造成心肌组织病理损伤,藏紫菀总黄酮可对抗缺氧引起的心肌组织损伤。与正常对照组比较,缺氧模型组小鼠心肌组织中MDA、H2O2含量及LDH、CK的活力均显著升高(P<0.01),HIF-1α和VEGF蛋白表达显著增强(P<0.01);与缺氧模型组比较,藏紫菀总黄酮组小鼠心肌组织中MDA、H2O2含量显著降低(P<0.05,P<0.01),LDH、CK的活力显著降低(P<0.01),HIF-1α和VEGF蛋白表达显著降低(P<0.05)。结论:藏紫菀总黄酮对高原缺氧引起的小鼠心肌组织损伤起到保护作用,其机制可能与减轻氧化应激及抑制缺氧诱导因子有关。 展开更多
