前房灌注联合玻璃体手术治疗伴有脉络膜脱离的视网膜脱离

目的评价前房灌注联合玻璃体切除术治疗伴有脉络膜脱离的视网膜脱离的效果。方法回顾性分析2017年1月至2018年2月在同济大学附属同济医院眼科就诊的伴有脉络膜脱离的视网膜脱离者16例(16只眼)的临床资料。所有患者在前房灌注的辅助下行... 目的评价前房灌注联合玻璃体切除术治疗伴有脉络膜脱离的视网膜脱离的效果。方法回顾性分析2017年1月至2018年2月在同济大学附属同济医院眼科就诊的伴有脉络膜脱离的视网膜脱离者16例(16只眼)的临床资料。所有患者在前房灌注的辅助下行玻璃体切除联合硅油填充术,术后随访6个月以上。结果术后1天至3个月所有患者视网膜均复位,复位率达100%。术后6个月,在硅油取出术中发现视网膜再次脱离1例,进行硅油置换。术后1、3及6个月,视力(LogMAR)依次为1.75±0.45、1.50±0.53、1.33±0.66,与手术前比较,差异有统计学意义(F=59.58,P=0.001)。术后1、3及6个月,眼压依次为(12.67±6.91)mmHg、(14.35±6.75)mmHg、(15.12±6.16)mmHg(1mmHg=0.133 kPa)。与术前比较,差异均有统计学意义(F=2.26,P=0.01)。结论玻璃体切除术联合前房灌注可有效治疗伴有脉络膜脱离的视网膜脱离。脉络膜及视网膜复位情况较好,视力有一定改善。 展开更多

Dopamine and its receptors have been widely studied in the neurological conditions and in the retina. In this study, we evaluated the possible role of dopamine in rhegmatogenous retinal detachment(RRD) by comparing the amount of 3,4-dihydroxyphenylacetic acid(DOPAC), a surrogate index of retinal dopamin levels, in the vitreous sample of patients affected by RRD with those affected by macular pucker and vitre ous hemorrhage. Our results showed that significantly higher levels of DOPAC were found in the vitreou sample of patients affected by RRD compared with those affected by vitreous hemorrhage and macula pucker(P = 0.002). Specifically, no trace of the substance was found in vitreous hemorrhage and macula pucker samples. A slightly significant positive correlation was found among DOPAC and post-operativ best corrected visual acuity(r = 0.470, P = 0.049). No correlation was found between DOPAC and the day elapsed between diagnosis and surgery(P = 0.317). For the first time our findings suggest that DOPAC i released in RRD, but not in other retinal diseases such as vitreous hemorrhage and macular pucker. More over, we showed a correlation between visual acuity outcome and the amount of DOPAC in the vitreous This might have a potential, although still unknown, implication in the pathogenesis of the disease and/o in the associated photoreceptors loss. The northwestern Sichuan region has experienced multi-stage tectonic evolution including marine cratonic basin from the Sinian to the Middle Triassic and intra-continental basin from the Late Triassic to the Cenozoic.... The northwestern Sichuan region has experienced multi-stage tectonic evolution including marine cratonic basin from the Sinian to the Middle Triassic and intra-continental basin from the Late Triassic to the Cenozoic. Several regional tectonic activities caused complicated stratigraphic distribution and structural deformations in the deep-buried layers. During the key tectonic periods, some characteristic sedimentary and deformation structures were formed, including the step-shaped marginal carbonate platform of Dengying Formation, the western paleo-high at the end of Silurian, and the passive continental margin of the Late Paleozoic–Middle Triassic. The Meso-Cenozoic intra-continental compressional tectonic processes since the Late Triassic controlled the formation of complex thrusting structures surrounding and inside the northwestern basin. The northern Longmenshan fold-thrust belt has a footwall in-situ thrust structure,which is controlled by two sets of detachments in the Lower Triassic and Lower Cambrian and presents as a multi-level deformation structure with the shallow folds, the middle thin-skin thrusts and the deeper basement-involved folds. The thrust belt in front of the Micangshan Mountain shows a double-layer deformation controlled by the Lower Triassic salt detachment, which is composed by the upper monocline and deep-buried imbricate thrust structures. The interior of the basin is characterized by several rows of large-scale basement-involved folds with NEE strike direction. From the perspective of structural geology, the favorable exploration reservoirs and belts in northwestern Sichuan have obvious zoning characteristics. The favorable exploration layers of Dengying Formation of Upper Sinian are mainly distributed in the eastern and northern areas of the northwestern Sichuan Basin, in which the Jiulongshan structural belt, Zitong syncline and Yanting slope are the most favorable. AIM: To assess the efficacy and safety of a heavy silicone oil (Densiron 68) in the management of inferior retinal detachment recurrence. METHODS: A retrospective non-comparative consecutive case series study. Forty-nine cases of complex inferior retinal detachment were treated using Densiron 68 heavy silicone oil (HSO) as the endotamponade. Our main purpose was anatomic reattachment following Densiron 68 removal. Functional outcomes, rate of recurrences, the presence of inflammatory complications and intraocular pressure alterations were evaluated. RESULTS: Forty-nine patients affected by complex retinal re-detachment were recruited. The mean follow-up was 7.6 (±1.5) mo. The mean best corrected visual acuity after Densiron 68 removal was 0.95 logMAR, standard error (SE: 0.068). Retinal reattachment was 61.2% after first surgery and 81.6% after second surgery. Nineteen cases (38.8%) had recurrences when intraocular heavy silicon oil was in situ, 26.3%(5 cases) of which involved the inferior retina. CONCLUSION: Densiron 68 ef?ciently fills the inferior retinal periphery and might lower the risk of inferior proliferative vitreoretinopathy development, in particular after a standard silicon oil tamponade that reduces the proliferative process in the upper quadrants of the retina. 展开更多

Chandelier-assisted scleral buckling using wide angle viewing contact lens for pseudophakic retinal detachment repair

AIM: To evaluate a modified technique for scleral buckling (SB) in pseudophakic retinal detachment (RD). METHODS: A retrospective non-comparative study included 21 consecutive eyes with uncomplicated pseudophakic RD that was repaired by chandelier assisted SB using wide angle viewing (WAV) contact lens. Segmental tire alone was used in 5 eyes (23.81%), and combined with encircling band in 7 eyes (33.33%). Radial sponge alone was used in 3 eyes (14.29%) and combined with encircling band in 6 eyes (28.57%). RESULTS: Primary success rate was (90.48%). External drainage of subretinal fluid was performed in 8 eyes (38.1%). Intraoperative complications included vitreous prolapse at chandelier sclerotomy site in 4 eyes (19.05%) and localized subretinal hemorrhage at drainage site in one eye (4.76%). No case of intraocular lens (IOL) displacement, retinal incarceration or iatrogenic retinal tear was detected. Postoperative complications included choroidal detachment in one eye (4.76%), elevated intraocular pressure in 2 eyes (9.52%), epiretinal membrane formation in one eye (4.76%) and proliferative vitreoretinopathy in 3 eyes (14.29%). Mean postoperative corrected distance visual acuity was 0.7±0.3 logMAR units. CONCLUSION: Chandelier-assisted SB using WAV contact lens is a reliable technique for repairing selected cases of simple pseudophakic RD. 展开更多

Abrupt visual loss during anti-vascular endothelial growth factor treatment for type 3 neovascularization

AIM: To investigate the incidence of abrupt visual loss and its associated factors, during anti-vascular endothelial growth factor(VEGF) treatment for type 3 neovascularization.METHODS: This retrospective study included 137 eyes that were newly diagnosed with type 3 neovascularization. All eyes were treated with anti-VEGF therapy. Abrupt visual loss was defined as loss of 5 or more lines in best-corrected visual acuity(BCVA) in comparison to the previous visit. The incidence and timing of abrupt visual loss as well as the factors associated with it, were determined. In addition, the BCVA at the final follow-up was compared between the eyes with and those without abrupt visual loss.RESULTS: The mean follow-up period was 42.4±18.9 mo after diagnosis, and abrupt visual loss was noted in 22 eyes(16.1%) at a mean of 19.6±13.9 mo. Abrupt visual loss was found to be associated with subretinal hemorrhage in 11 eyes(50.0%), development of or increase in the height of pigment epithelial detachment with fluid in 8 eyes(36.4%), and tears in the retinal pigment epithelium in 3 eyes(13.6%). The logarithm of minimum angle of resolution(logMAR) mean BCVA at the final follow-up was 2.07±0.67(Snellen equivalents: 20/2349) and 1.00±0.55(20/200) in eyes with and without abrupt visual loss, respectively. BCVA was significantly worse in the eyes with abrupt visual loss(P<0.001).CONCLUSION: Abrupt visual loss is noted in 16.1% of patients with type 3 neovascularization and is associated with poor visual outcome. Additional studies are needed to determine how abrupt visual loss can be prevented. 展开更多

Treatment of Coats’ disease: an analysis of pooled results

AIM: To elucidate the association of treatment modality to vitreoretinal fibrosis and traction retinal detachment (TRD) in Coats’ disease. METHODS: A PubMed search for Coats’ disease with included studies describing eyes with clinical features and treatment course of Coats’ disease. Binary logistic regression with fibrosis at presentation and treatment type as independent variables was performed to determine predictors of TRD historically (since 1921) and in the anti-vascular endothelial growth factor (VEGF) era (since 2007). Odds ratios (OR) with 95% confidence intervals (CI) reported. RESULTS: Of 175 articles described 1183 eyes. Vitreoretinal fibrosis increased from presentation (5.4%) to follow-up (15.5%) and TRD increased from 0.44% to 3.9% at follow up. Laser was protective against vitreoretinal fibrosis (OR 0.6, 95%CI 0.4-0.9) but TRD was borderline (OR 0.6, 95%CI 0.3-1.1). Cryotherapy showed a higher association with TRD (OR 1.9, 95%CI 1.0-3.7) than with vitreoretinal fibrosis (OR 0.8, 95%CI 0.5-1.2). Similarly, intravitreal anti-VEGF alone was not associated with fibrosis (OR 1.1, 95%CI 0.6-1.8) nor TRD (OR 1.1, 95%CI 0.5-2.6) but the combination of laser and anti-VEGF therapy was protective [Fibrosis: 0.1 (0.03, 0.35);TRD: 0.05 (0.01, 0.23)] compared to anti-VEGF plus cryotherapy (P<0.001). Disease stage ≤2B or ≥3A was not associated with TRD. CONCLUSION: Vitreoretinal fibrosis and TRD increase after treatment in Coats’ disease. The combination of anti-VEGF agents and cryotherapy may lead to higher risk for TRD. Presence of pre-treatment fibrosis is the highest risk factor for post-treatment worsening of vitreoretinal fibrosis and TRD. 展开更多

研究目的:新建地铁隧道上穿施工将引起既有隧道内的轨道结构发生变形甚至脱空,影响既有地铁的安全运营。为此,将轨道结构视为弹性地基梁,在控制微分方程中引入阶梯函数,推导考虑轨道与基底脱空的轨道变形傅里叶级数解法,并将理论方法与... 研究目的:新建地铁隧道上穿施工将引起既有隧道内的轨道结构发生变形甚至脱空,影响既有地铁的安全运营。为此,将轨道结构视为弹性地基梁,在控制微分方程中引入阶梯函数,推导考虑轨道与基底脱空的轨道变形傅里叶级数解法,并将理论方法与数值模拟方法及实测数据进行对比,验证方法的正确性,进而基于傅里叶级数法进行参数分析。研究结论:(1)轨道变形、内力和脱空范围均随隧道隆起量的增大而增大,轨道结构内力和脱空范围随隧道上浮槽宽度的增大而减小;(2)当双洞隧道中距大于8倍的上浮槽宽度时,可按单洞隧道确定基底变形;(3)当双洞隧道中距小于4倍的上浮槽宽度时,道床板的最大隆起将超过隧道底板;(4)本研究成果可为隧道上穿施工中既有轨道结构的安全评价提供理论依据。 展开更多

The multi-orifice plate gas sparger, mainly composed of a multi-orifice plate and a gas chamber, is one of the most common sparger facilities.The aeration performance of multi-orifice plate has a close relation with the multiorifice plate configuration.In addition, the weeping phenomenon has a considerable influence on the gas chamber condition which affects the bubble detachment volume directly.This paper conducts a set of visual experiments to study the influence of multi-orifice configuration and gas chamber condition on the aeration performance of gas sparger.For multi-orifice plate, an improved theoretical model is proposed which considers the wave effect of the previous bubbles generated from adjacent orifices and the variance of the number of active bubbling orifice.A parameter is proposed to evaluate the aeration performance in order to overcome the difficulty caused by the randomness of bubble formation process.The experimental results suggest that the gas chamber filled with water is in favor of large bubble formation.The influence of the pitch of orifice on aeration performance can only be observed in high-restricted case.According to the theoretical model and experimental results, the influences of gas flow rate and the number of open orifices on the aeration performance are analyzed and a design criterion for the number of open orifice is proposed. 展开更多

Based on the volume of fluid(VOF) method, we conduct a numerical simulation to study the hydrodynamic binary coalescence of droplets under air flow in a hydrophobic rectangular microchannel. Two distinct regimes, coalescence followed by sliding motion and that followed by detaching motion, are identified and discussed. Additionally, the detailed hydrodynamic information behind the binary coalescence is provided, based on which a dynamic mechanical analysis is conducted to reveal the hydrodynamic mechanisms underlying these two regimes. The simulation results indicate that the sliding motion of droplets is driven by the drag force and restrained by the adhesion force induced by the interfacial tension along the main flow direction. The detachment(i.e., upward motion) of the droplet is driven by the lift force associated with an aerodynamic lifting pressure difference imposed on the coalescent droplet, and also restrained by the adhesion force perpendicular to the main flow direction. Especially, the lift force is mainly induced by an aerodynamic lifting pressure difference imposed on the coalescent droplet. Two typical regimes can be quantitatively recognized by a regime diagram depending on Re and We. The higher Re and We respectively lead to relatively larger lift forces and smaller adhesion forces acting on the droplet, both of which are helpful to detachment of the coalesced droplet. 展开更多

目的分析白内障手术人工晶状体植入术后出现视网膜脱离(RD)的危险因素。方法回顾性分析2010年6月至2016年5月本院收治的白内障3 548例(3 548眼)的临床资料,随访时间2年。分析术中医源性因素、年龄、眼轴长度及近视等因素与术后视网视脱... 目的分析白内障手术人工晶状体植入术后出现视网膜脱离(RD)的危险因素。方法回顾性分析2010年6月至2016年5月本院收治的白内障3 548例(3 548眼)的临床资料,随访时间2年。分析术中医源性因素、年龄、眼轴长度及近视等因素与术后视网视脱离的关系。结果术后发生RD 36例,占所有白内障手术的1.01%;男性比例较高;眼轴小于24 mm的RD患者发生率较低,而眼轴为27-28 mm的RD患者发生率高达3.59%;白内障手术后RD发生率在50?59岁人群中发生率最高,平均年龄为(58.56 ±9.9)岁,随着年龄的增加RD发生率显著降低。术中并发晶状体后囊破裂、后发性白内障行激光后囊切开术与视网膜脱离密切相关。结论白内障术后视网膜脱离与性别、年龄、眼轴长度、高度近视、术中晶状体后囊破裂及后发性白内障行激光后囊切开术密切相关。 展开更多

目的观察玻璃体视网膜手术治疗激光后囊切开术后视网膜脱离的效果。方法回顾性病例分析。Nd:YAG激光后囊切开后视网膜脱离30例(30眼),在Resight非接触广角镜系统下进行玻璃体视网膜手术。随访12个月。结果一次手术后视网膜复位27例(90.0... 目的观察玻璃体视网膜手术治疗激光后囊切开术后视网膜脱离的效果。方法回顾性病例分析。Nd:YAG激光后囊切开后视网膜脱离30例(30眼),在Resight非接触广角镜系统下进行玻璃体视网膜手术。随访12个月。结果一次手术后视网膜复位27例(90.00%)。未复位的3例中,1例进行了2次硅油填充术,1例进行了3次硅油填充术后视网膜得以复位;另外1例因为严重的PVR导致视网膜复位困难而放弃治疗。最后视网膜复位为29例(96.67%)。视力(Log MAR)术后各时间点与术前比较差异均具有统计学意义(F=2.468,P=0.001)。术后12个月视力最好。结论23 G玻璃体切除术治疗激光后囊切开术后视网膜脱离安全有效。 展开更多
