BI-RADS联合超声弹性成像对非哺乳期乳腺炎及乳腺癌的鉴别价值 认领

目的:探讨乳腺影像报告和数据系统(Breast Imaging Reporting and Data System,BI-RADS)联合超声弹性成像(ultrasound elastography,UE)对非哺乳期乳腺炎(non-lactating mastitis,NLM)及乳腺癌的鉴别价值。方法:回顾性选取2018年1月至2... 目的:探讨乳腺影像报告和数据系统(Breast Imaging Reporting and Data System,BI-RADS)联合超声弹性成像(ultrasound elastography,UE)对非哺乳期乳腺炎(non-lactating mastitis,NLM)及乳腺癌的鉴别价值。方法:回顾性选取2018年1月至2019年1月某院经病理证实的非哺乳期乳腺肿块患者159例,其中乳腺癌50例、NLM 109例。所有患者均给予常规超声(采用BI-RADS评分)、UE检查,比较超声检查情况以及BI-RADS、UE评分结果,以病理检查为对照,分析BI-RADS、UE及二者联合对NLM及乳腺癌的鉴别价值。采用SPSS 22.0软件进行数据分析。结果:NLM和乳腺癌患者边缘不完整占比比较差异无统计学意义(P>0.05),乳腺癌患者BI-RADS、UE评分及后方回声衰减、钙化、内部血流Ⅱ级和Ⅲ级占比明显高于NLM患者,差异有统计学意义(P<0.05)。在鉴别NLM及乳腺癌的敏感度、特异度、准确度中,BI-RADS为80.00%、82.57%、81.76%,UE为76.00%、78.90%、77.99%,BI-RADS联合UE为96.00%、93.58%、94.34%,BI-RADS联合UE明显高于BI-RADS或UE单独诊断,差异有统计学意义(P<0.05)。结论:BI-RADS、UE对NLM及乳腺癌具有良好的鉴别价值,且BI-RADS联合UE鉴别价值更高,值得临床推广。 展开更多

Goiter is an enlargement of the thyroid gland which can be associated with a number of complications both for the mother and the fetus. A 34-year-old pregnant woman with normal thyroid function was referred to our Department of Obstetrics and Gynecology at Microcitemico Pediatric Hospital, Cagliari, for suspected fetal goiter at 32 gestational weeks. The case was monitored regularly by ultrasound and treated successfully with intra-amniotic levothyroxine (L-T4) administration. Fetal goiter was observed to decrease after this treatment and the thyroid ultrasound findings were also normal both at birth and in subsequent follow-ups. Our case report confirms the feasibility of conservative treatment with L-T4, which can effectively prevent complications related to fetal goiter. 展开更多

Botulinum Toxin (BT) infiltration is the treatment of choice for CD, according to most of the experts around the world, however the efficacy and tolerance of BT therapy in CD depend on the accuracy when BT is released into the muscles. We reviewed the medical literature in regard to the use of guiding tools for the BT infiltration in CD patients. Results: The use of guiding tools such as Ultrasound or EMG definitely improves the accuracy for releasing the BT into the muscles involved according to some authors. Conclusion: the use of Ultrasound and EMG improves the efficacy and reduce the adverse effects in the BT therapy in CD patients. 展开更多

背景:间充质干细胞是目前组织工程和再生医学研究的热点之一,然而如何在体外进行干细胞的有效扩增以满足临床需求仍是研究者们面临的难题。目的:探讨低强度脉冲场超声对人脂肪间充质干细胞增殖和黏附的影响。方法:取第3代人脂肪间充质... 背景:间充质干细胞是目前组织工程和再生医学研究的热点之一,然而如何在体外进行干细胞的有效扩增以满足临床需求仍是研究者们面临的难题。目的:探讨低强度脉冲场超声对人脂肪间充质干细胞增殖和黏附的影响。方法:取第3代人脂肪间充质干细胞,在人脂肪间充质干细胞培养24,48,72 h时以0,10,30,50 mW/cm~2低强度脉冲场超声强度进行第1,2,3次刺激,每次刺激时间为5 min,刺激结束后将细胞置于培养箱继续培养24 h,然后采用CCK-8实验筛选最佳刺激条件。选用30 mW/cm~2强度刺激2次后的细胞为实验组进行后续实验,0 mW/cm~2刺激为对照组。流式细胞仪术检测细胞表面标志物;免疫组化染色观察细胞增殖相关抗原Ki-67和黏着斑激酶FAK的表达;RT-PCR检测细胞增殖通路相关基因Cyclin D1和c-myc的表达;转录组测序检测转录水平基因的变化。结果与结论:①30 mW/cm~2强度连续刺激2次是促进人脂肪间充质干细胞增殖的最佳条件;②与对照组相比,30 mW/cm~2低强度脉冲场超声刺激后细胞表面标志物未改变,增殖相关基因CyclinD1、c-myc和黏着斑激酶FAK表达上调;③转录组测序结果显示有27个基因出现显著性差异,其中15个基因上调,12个基因下调;④结果表明,低强度脉冲场超声通过上调CyclinD1、c-myc基因和FAK的表达,伴随转录组多个基因的调控,在细胞未分化的状态下,促进人脂肪间充质干细胞增殖。 展开更多

Th... <div style="text-align:justify;"> The morbidity and mortality of the fetus is related closely with the neonatal respiratory morbidity, which was caused by the immaturity of the fetal lung primarily. The amniocentesis has been used in clinics to evaluate the maturity of the fetal lung, which is invasive, expensive and time-consuming. Ultrasonography has been developed to examine the fetal lung quantitatively in the past decades as a non-invasive method. However, the contour of the fetal lung required by existing studies was delineated in manual. An automated segmentation approach could not only improve the objectiveness of those studies, but also offer a quantitative way to monitor the development of the fetal lung in terms of morphological parameters based on the segmentation. In view of this, we proposed a deep learning model for automated fetal lung segmentation and measurement. The model was constructed based on the U-Net. It was trained by 3500 data sets augmented from 250 ultrasound images with both the fetal lung and heart manually delineated, and then tested on 50 ultrasound data sets. With the proposed method, the fetal lung and cardiac area were automatically segmented with the accuracy, average IoU, sensitivity and precision being 0.98, 0.79, 0.881 and 0.886, respectively. </div> 展开更多

Excellent firm bonding between the biomaterials and bone tissue (osseointegration and osteo-conductivity) has been desired for the stability in vivo of dental implants and artificial joints. Much has been learned about this concept, which has led to significant improvements in the design and surface modification of implants in the field of implant dentistry, orthopedic surgery. We have already reported that low-intensity pulsed ultrasound (LIPUS) irradiation can accelerate the bone bonding ability of the bio-conductive materials such as bioactive titanium and hydroxyapatite implant. However, it is still unclear whether the LIPUS could have same effect to different types of the bioactive-materials. Therefore, in this study, the differences of bone-like hydroxyapatite formation on some kind of hydroxyapatite surface in simulated body fluid (SBF) under the LIPUS irradiation were investigated. Two kinds of hydroxyapatite samples immersed in SBF was exposed to ultrasound waves, the bone-like apatite on the surface was analyzed by Scanning electron microscopy and X-ray diffraction. As a result, the enhancement of hydroxyapatite formation on the surface by LIPUS was confirmed, the initial epitaxial nucleation and crystal growth of apatite depended on crystal structure of the surface of matrix materials. 展开更多

BACKGROUND Endoscopic ultrasound-guided fine needle aspiration or biopsy(EUS-FNA or FNB)has become a popular method for diagnosing various lesions of the gastrointestinal tract and surrounding tissue due to the accuracy and safety.To the best of our knowledge,no case report of severe infection after EUS-FNB of a solid lesion in the spleen has been described.Herein,we report a rare case of septic shock after EUS-FNB of a splenic mass.CASE SUMMARY A 45-year-old male patient presented to the outpatient clinic due to an incidentally detected splenic mass.A definitive diagnosis could not be established based on the abdominal magnetic resonance imaging.EUS of the spleen showed a 6 cm-sized,relatively well-demarcated,heterogeneous mass,and EUS-FNB with a 22G needle was performed.Ten days after the procedure patient developed septic shock and a splenic abscess was identified.Blood culture revealed growth of Granulicatella adiacens.After the treatment with antibiotics the patient underwent surgical resection,and the pathological examination showed diffuse large B-cell lymphoma.The patient received chemotherapy and he is in complete remission.CONCLUSION Infection of a splenic mass after EUS-FNB is a rare complication and prophylactic antibiotics might be considered. 展开更多

对比MRI和超声诊断剖宫产术后子宫瘢痕妊娠的临床价值 认领

目的探讨MRI和超声诊断剖宫产术后子宫瘢痕妊娠(CSP)的临床价值。方法回顾性分析我院经手术病理证实的48例CSP患者的临床资料及MRI和超声影像学特征。结果超声诊断正确43例,误诊5例,诊断准确率为89.6%;MRI诊断正确46例,误诊2例,诊断准确... 目的探讨MRI和超声诊断剖宫产术后子宫瘢痕妊娠(CSP)的临床价值。方法回顾性分析我院经手术病理证实的48例CSP患者的临床资料及MRI和超声影像学特征。结果超声诊断正确43例,误诊5例,诊断准确率为89.6%;MRI诊断正确46例,误诊2例,诊断准确率95.8%。切口瘢痕位于子宫前壁下段呈长T1短T2信号,增强无强化;孕囊表现为水滴状、卵圆形或腊肠样长T1长T2为主信号,增强扫描囊壁呈环形、非闭环性强化,部分囊壁不强化,瘢痕周围子宫肌层呈小片状、结节样或不均匀强化;孕囊突向腹腔则表现为子宫峡部前壁肌层明显变薄,甚至穿透子宫浆膜层累及膀胱后壁,与周围结构分界不清。MRI对评价孕囊及宫腔出血明显优于超声,但对卵黄囊的显示及判断胚胎是否存活较超声明显不足。对比MRI和超声诊断剖宫产术后子宫瘢痕妊娠的准确率比较无统计学差异性(χ^(2)=1.387,P>0.05)。结论MRI和超声对剖宫产术后子宫瘢痕妊娠均有较高的诊断价值,CSP MRI影像学征象具有一定特异性,能为临床诊断及个性化治疗提供帮助。 展开更多

Performing an ultrasound scan at the slightest clinical suspicion contributes to early diagnosis and therapeutic management. <strong>Objective:</strong> To show the relevance of ultrasound in the therapeutic management of the child’s intussusception through a correlation between the ultrasound diagnosis and the clinical and/or per operative diagnosis. <strong>Patients and Methods:</strong> It was a retrospective study of 24 cases from July 2017 to September 2020 (30 months) in the Medical Imaging and Paediatric Surgery departments of Teaching Hospital of Bouaké. We included only patients from 0 to 15 years old who had digestive symptoms, an abdominal ultrasound scan. These patients were eligible for surgery. Data analysis was performed with Epi info 7 software. <strong>Results:</strong> Median age was 17.2 months [02 - 120]. Male gender predominated (83.3%). Clinico-biological data were dominated by abdominal pain (79.2%), vomiting (75%) and rectorragies (33.3%), with Ombredanne’s triad in 33.3% and hyperleukocytosis (70.8%). Ultrasonography showed invaginated coves (95.8%) sitting in the right angl iliac fossa, peri-umbilical and right flank in 73.9%. Cockade image coupled with the sandwich image accounted for 95.6%. Superficial adenopathies (45.8%) and peri-lesional fluid effusion (20.8%) were associated with it. An occlusive syndrome complicating intussusception was observed in 41.67% associated with intra-peritoneal effusion of fluid (25%). There was a significant diagnostic agreement between the clinic, ultrasound and surgery according to the Fisher’s exact test (p = 0.002). That between ultrasound and surgery was calculated at 95.4%. <strong>Conclusion:</strong> This study allowed us to show a good clinical-echo-surgical concordance. Ultrasound is therefore an undeniable diagnostic, prognostic and therapeutic tool in the exploration of intussusception, hence the interest of its prescription in the shortest possible time by the clinician. 展开更多

We report the clinical case of a myxom... <strong>Introduction</strong>: Cardiac myxomas represent the most frequent forms of primary tumors of the heart. The most frequent location is the interatrial septum. We report the clinical case of a myxoma of the left atrium and discuss its epidemiological and therapeutic aspects through a review of the literature. <strong>Observation</strong>: This was a 41-year-old female patient who presented with sudden rotational dizziness associated with vomiting. MRI revealed multiple punctiform bilateral supra and subtentorial strokes of different ages, recent and semi-recent, suggesting an embologenic etiology. Transesophageal echocardiography found a large pedunculated, homogeneous, avascular tumor hanging from the interatrial septum. The patient is operated on urgently under cardiopulmonary bypass for resection of a large tumor located in the left atrium. The pathological examination concluded with the diagnosis of myxoma of the left atrium. The postoperative follow-up was straightforward and the patient was discharged from the hospital via home hospitalization. <strong>Conclusion</strong>: The diagnosis of cardiac myxomas is suspected in the presence of symptoms associated with echocardiographic images of intracardiac masses and confirmed by histological study. Embolic accidents are one of the formidable complications of myxomas. Surgical management is urgent, especially in the presence of predictive morphological features of embolism on echocardiography. 展开更多

The study was conducted to determine the role of radiological investigation in assessing the severity of COVID-19 pneumonia with the help of chest X-ray (CXR), high resolution computed tomography (HRCT), and ultrasound. This was a retrospective study conducted on 105 COVID-19 patients with symptoms of fever, cough, dyspnea, loss of sense of smell and taste, body ache. Diagnostic tests PCR were positive for COVID-19 included from Medicare Cardiac and General Hospital and Jinnah Medical College Hospital Korangi (JMCH) from April to November 2020. Written informed consent was taken from all participants. This study was approved by ethical review committee, Jinnah Medical & Dental College. Real time-PCR (RT-PCR) was done for the confirmation. Radiological imaging including Chest X-ray, HRCT chest, and ultrasound was done to study the severity of symptoms. Sixty-five patients had mild to moderate symptoms with oxygen saturation between 96% - 98% and 30 patients with severe pneumonia had between saturation 50% - 55%. Patients with mild symptoms were followed up by chest X-ray showing mostly normal chest X-rays but single or patches of ground glass opacities (HRCT). HRCT of 10 patients with low oxygen saturation 50% were already done showing bilateral peripheral patchy consolidation predominantly involving mid and lower lobes. 3 patients presented with patchy lung opacities and ultrasound showing similar findings with pleural effusion and ascites. Radiological imaging, specially CT-Scan was highly significant for diagnosing COVID-19 and severity of infection even in patients with negative PCR. 超声波预处理对玉米醇溶蛋白结构及其Pickering乳液稳定性的影响 认领

为探究超声波预处理在反溶剂法制备玉米醇溶纳米颗粒(zein nanoparticle,ZNP)过程中的作用,并将ZNP通过静电作用与亚麻籽胶(flaxseed gum,FSG)结合形成复合颗粒,探究其对Pickering乳液稳定性的影响,对不同超声波处理条件对ZNP的内源荧... 为探究超声波预处理在反溶剂法制备玉米醇溶纳米颗粒(zein nanoparticle,ZNP)过程中的作用,并将ZNP通过静电作用与亚麻籽胶(flaxseed gum,FSG)结合形成复合颗粒,探究其对Pickering乳液稳定性的影响,对不同超声波处理条件对ZNP的内源荧光性、二级结构、粒径和电位的影响进行分析。结果表明,超声波预处理玉米醇溶蛋白的乙醇水溶液,可以显著降低ZNP的平均粒径,并提高ZNP的电位绝对值。在较低超声功率密度(230 W/cm2)下,粒径随着处理时间延长而进一步减少,电位绝对值增加,即稳定性提升。但当超声功率密度增加至460 W/cm2时,ZNP的粒径开始缓慢增大,电位绝对值降低。将经超声波预处理的ZNP与FSG按质量比1∶1均质混合,通过表面疏水性分析发现:由超声波预处理的ZNP可以与更多的FSG结合,提高亲水性,分析其原因可能是ZNP表面电位升高,促进了更多FSG结合在ZNP的表面,有效地抵御乳液中液滴的聚集及破裂。通过对热稳定性的研究发现,超声波预处理可以显著提升Pickering乳液的稳定性。该研究结果可为超声波预处理在ZNP及Pickering乳液制备过程中的应用提供一定的基础依据,并为制备更高效、热稳定的Pickering粒子提供一种新型绿色的途径。 展开更多
