AIM: To identify the risk factors for organ failure (OF)in cholangitis with bacteriobilia.METHODS: This study included 182 patients withacute cholangitis who underwent percutaneoustranshepatic biliary drainage between January 2005and April 2013. We conducted a retrospective analysisof comprehensive clinical and laboratory data.RESULTS: There were 24 cases (13.2%) of OF andfive deaths (2.7%). Bile culture was positive formicrobial growth in 130 out of 138 (94.2%) patients.In multivariate analysis of 130 patients with positive bile cultures, significant predictive factors for OF werethe presence of extended-spectrum beta-lactamase(ESBL) organisms in blood cultures, pre-existing renaldysfunction, and choledocholithiasis as an etiology, withodds ratios of 15.376, 6.319, and 3.573, respectively.We developed a scoring system with a regressioncoefficient of each significant variable. The OF scorewas calculated using the following equation: (2.7 ×ESBL organisms in blood cultures) + (1.8 × pre-existingrenal dysfunction) + (1.3 × choledocholithiasis). Thisscoring system for predicting OF was highly specific(99.1%) and had a positive predictive value of 86.2%.CONCLUSION: ESBL organisms in blood cultures, preexistingrenal dysfunction, and choledocholithiasis arerisk factors for OF in cholangitis with bacteriobilia. TheOF scoring system may aid clinicians to identify a poorprognosis group. 展开更多

Objective:To evaluate inhibitoy potential of seven Korean thistles against the advanced glyeatinn endproducts(AGE) formation as well as to identify responsible compounds from the most active species.Methods:We used an in vitro AGE inhibition assay to evaluate the antidiabetic complication potential of the methanol extracts of the selected Korean thistles.Results:Among the seven Korean thistles,the leaves of Cirsium maackii(C.maackii) exhibited the most significant inhibitory activity against AGE formation.By means of bioassay-directed fractionation,a lignan.chlorogenic acid and 14 flavonoids were isolated from the active ethyl acetate soluble fraction of a methanol extract from C.maackii leaves.Luteolin and its 5-O-glueoside have been previously isolated:however,a lignan and 13 known compounds were isolated for the first lime from C maackii leaves in this study.Most of the isolated compounds exhibited inhibitory activities against potential AGE formation.Among them,cernuoside was shown to be the most potent AGE inhibitor with an IC<sub>50</sub> value of 21.21 μ mol/L.Most importantly,two major flavonoids,luteolin and ils 5-O-glucoside,also significantly inhibited AGE formation,with IC<sub>50</sub> values of36.33 and 37.47 μmol/L,respectively.Structure activity relationship revealed that the presence of free 3’and 4’ dihydroxyl group in flavonoids skeleton played an important role in AGE inhibition.Conclusions:These results indicate that C.maackii and C maackii-derived flavonoids might be explored further to develop therapeutic agents for the prevention of diabetic eoniplicalions due to their significant inhibitory activity against AGE formation. 展开更多

A terminal and location, without limitations on time, are continuously available to the environment of a high- quality knowledge content was coming through a variety of smart media, open educational content platform is built over the level of the possible conditions than e-learning. In this paper, we develop applications and Web sites that can provide authored content for smart cloud learning. In addition, we have built the cloud for content management and the website that can share content with other users. AIM:To evaluate the anti-tumor effect of clobenpropit,which is a specific H3 antagonist and H4 agonist,in combination with gemcitabine in a pancreatic cancer cell line.METHODS:Three kinds of human pancreatic cancer cell lines(Panc-1,MiaPaCa-2,and AsPC-1)were used in this study.Expression of H3 and H4 receptors in pancreatic cancer cells was identified with Western blotting.Effects of clobenpropit on cell proliferation,migration and apoptosis were evaluated.Alteration of epithelial and mesenchymal markers after administration of clobenpropit was analyzed.An in vivo study with a Panc-1xenograft mouse model was also performed.RESULTS:H4 receptors were present as 2 subunits in human pancreatic cancer cells,while there was no expression of H3 receptor.Clobenpropit inhibited cell migration and increased apoptosis of pancreatic cancer cells in combination with gemcitabine.Clobenpropit up-regulated E-cadherin,but down-regulated vimentin and matrix metalloproteinase 9 in real-time polymerase chain reaction.Also,clobenpropit inhibited tumor growth(gemcitabine 294±46 mg vs combination 154±54 mg,P=0.02)and enhanced apoptosis in combination with gemcitabine(control 2.5%,gemcitabine25.8%,clobenpropit 9.7%and combination 40.9%,P=0.001)by up-regulation of E-cadherin and downregulation of Zeb1 in Panc-1 xenograft mouse.CONCLUSION:Clobenpropit enhanced the anti-tumor effect of gemcitabine in pancreatic cancer cells through inhibition of the epithelial-mesenchymal transition process. 展开更多

We report a case of successful survival from ventricular free wall rupture without surgery, which was not overt rupture but micro-rupture. The patient was diagnosed with acute myocardial infarction (AMI) and underwent primary percutaneous coronary intervention (PCI). Myocardial blood leakage could be detected on transthoracic echocardiography with intravenous infusion of contrast dye 3 days after PCI. This examination revealed that there was not overt perforation but diffuse fissure. Because of his critical condition, he was treated conservatively without surgery. AIM: To identify clinical and pathological differences between serum immunoglobulin G4 (IgG4)-positive (SIP) and IgG4-negative (SIN) type 1 autoimmune pancreatitis (AIP) in South Korea. METHODS: AIP was diagnosed by ... AIM: To identify clinical and pathological differences between serum immunoglobulin G4 (IgG4)-positive (SIP) and IgG4-negative (SIN) type 1 autoimmune pancreatitis (AIP) in South Korea. METHODS: AIP was diagnosed by the international consensus diagnostic criteria. The medical records and pathology were retrospectively reviewed and IgG4-positive cells were counted in a high power field (HPF). Type I AIP was defined as a high serum level of IgG4or histological finding. SIN type 1 AIP was defined as a histological evidence of type 1 AIP and a normal serum IgG4 level. The clinical and pathological findings were compared between the two groups. The analysis was performed using Student’s t test, Fischer’s exact test and Mann-Whitney’s U test. A P value of 【 0.05 was considered statistically significant. As repeated com- parison was made, P values of less than 5% (P 【 0.05) were considered significant. RESULTS: Twenty five patients with definite type 1 AIP (19 histologically and six serologically diagnosed cases) were enrolled. The mean tissue IgG4 concentrations were significantly higher in SIP than SIN group (40 cells per HPF vs 18 cells per HPF, P = 0.02). Among eight SIN patients, the tissue IgG4 concentrations were less than 15 cells per HPF in most of cases, except one. The sensitivity of serum IgG4 was 68% (17 SIP and eight SIN AIP). Other organ involvement was more frequent- ly associated with SIP than SIN AIP (59% vs 26%, P = 0.016). However, the relapse rate and diffuse swelling of the pancreas were not associated with serum IgG4 level. The concentrations of IgG4-positive cells per HPF were higher in SIP than SIN AIP (40 vs 18, P = 0.02). CONCLUSION: The sensitivity of serum IgG4 was 68% in type 1 AIP. High serum IgG4 level was associated with other organ involvement and tissue IgG4 concentration but did not affect the relapse rate in type 1 AIP. 展开更多
