摘要
目的:探讨儿童新型冠状病毒肺炎(novel coronavirus pneumonia,NCP)的临床特征及CT影像表现。方法:回顾性搜集2020年1月20日至2月15日十堰市太和医院经临床及核酸检测确诊的20例NCP患儿的临床及胸部CT影像资料。其中男6例,女14例,年龄2个月~10岁,中位年龄2岁9月,5岁以下患儿14例。分析20例NCP患儿的临床特征、实验室检查以及胸部CT影像学表现。结果:20例患儿胸部高分辨率CT(HRCT)图像均有渗出性病灶,表现如下:11例为肺野中外带分布的斑片状、结节状磨玻璃影或实变影,其中9例为胸膜下分布;2例为节段性实变影与磨玻璃影共存;2例为肺内局限性沿着小血管分布的磨玻璃影;5例为类小叶性肺炎特点,表现为沿支气管、血管束分布的结节样、小斑片样影。结论:儿童NCP临床症状不典型,其HRCT影像表现多样,在特定的流行病学背景下,及时开展胸部HRCT扫描,可对疾病做出早期预判。
Objective To explore the clinical and CT imaging characteristics of novel coronavirus pneumonia(NCP)in children.Methods A retrospective analysis was performed on clinical and the chest HRCT findings of 20 children diagnosed with NPC by clinical and nucleic acid tests in Shiyan Taihe Hospital from January 20 to February 15,2020.There were 6 males and 14 females,aged from 2 months to 10 years,with a median age of 2 years and 9 months,and 14 children under 5 years old.The clinical features,laboratory examination indexes and HRCT findings of 20 children with NCP were analyzed.Results There were exudative lesions on the chest HRCT images in 20 children.The manifestations were as follows:11 cases were patchy,nodular ground glass shadow or consolidation shadow distributed in the middle and outer zone of the lung field,of which 9 cases were subpleural distribution and 2 cases were coexistence of segmental consolidation shadow and ground glass shadow.The coexistence of segmental consolidation and ground glass shadow was found in 2 cases;2 cases showed ground glass shadows with localized distribution along small blood vessels in the lung;5 cases were characterized by lobular pneumonia,showing nodular and patchy shadows distributed along the bronchi and vascular bundles.Conclusion The clinical symptoms of children with NCP are atypical and their HRCT findings are diverse.Under the specific epidemiological background of NCP,HRCT scan of chest should be carried out in time to make early prediction of the disease.
