环抱式爪形肋骨接骨板治疗连枷胸的临床效果 预览

摘要 目的探讨环抱式爪形肋骨接骨板治疗连枷胸的临床效果。方法选取2015年1月至2018年1月我院收治的47例连枷胸患者为研究对象,根据治疗方式的不同将其分为外固定组(22例,传统外固定包扎法治疗)与内固定组(25例,环抱式爪形肋骨接骨板治疗),比较两组患者的治疗效果。结果内固定组的ICU住院时间、总住院时间、机械通气时间及拔管时间均短于外固定组,并发症总发生率低于外固定组(P<0.05)。治疗后1个月,内固定组的FVC、PEF、FEV1/FVC、MMEF均显著高于外固定组(P<0.05)。T2时,两组患者的CI、CVP及HR均优于T1时,且内固定组优于外固定组(P<0.05);T3时,两组患者的CI、CVP及HR均优于T1和T2时,且内固定组优于外固定组(P<0.05)。治疗后,两组的APACHEⅡ评分均降低,且内固定组低于外固定组(P<0.05)。结论环抱式爪形肋骨接骨板治疗连枷胸效果显著,可改善患者的肺功能、血流动力学及生命质量,降低患者的并发症发生率,值得临床推广与应用。 Objective To explore the clinical effect of embracing claw-shaped rib grafting plate in the treatment of flail chest. Methods Forty-seven flail chest patients admitted in our hospital from January 2015 to January 2018 were selected as the research objects. According to the different treatment methods, the patients were divided into external fixation group(22 cases, traditional external fixation bandaging therapy) and internal fixation group(25 cases, embracing claw-shaped rib grafting plate therapy). The therapeutic effects of the two groups were compared. Results The ICU hospitalization time, total hospitalization time, mechanical ventilation time and extubation time in the internal fixation group were shorter than those in the external fixation group, and the total incidence of complications was lower than that in the external fixation group(P <0.05). One month after treatment, FVC, PEF, FEV1/FVC and MMEF in the internal fixation group were significantly higher than those in the external fixation group(P<0.05). At T2, the CI, CVP and HR of the two groups were better than those at T1, and those in the internal fixation group were better than the external fixation group(P<0.05);at T3, the CI, CVP and HR of the two groups were better than those at T1 and T2, and those in the internal fixation group were better than the external fixation group(P <0.05). After treatment, the APACHEE Ⅱ scores of both groups decreased, and that in the internal fixation group was lower than the external fixation group(P<0.05). Conclusion The treatment of flail chest with embracing claw-shaped rib grafting plate has significant effect, it can improve the pulmonary function, hemodynamics and quality of life of patients, and reduce the incidence of complications, which is worthy of clinical promotion and application.
