骨瓣开颅显微镜术式与神经外科锁孔术对颅内肿瘤患者的疗效对比 预览 被引量:11

Efficacy of Bone Flap Craniotomy and Keyhole Surgery of Neurosurgery on Patients with Intracranial Tumors

摘要 目的:研究骨瓣开颅显微术式与神经外科锁孔术对颅内肿瘤病患的治疗效果。方法:选择2013年1月至2015年4月进行治疗的颅内肿瘤病患154例,以数字法随机分为观察组和对照组各77例。其中观察组采用神经外科锁孔术治疗,对照组采用骨瓣开颅显微镜术式治疗。对比两组手术情况,同时对所有的治疗效果、生活质量进行评定。结果:观察组手术时间及术中出血量均显著低于对照组;观察组治愈率、治疗总有效率以及2年内存活率分别为63.64%(49/77)、96.10%(74/77)、87.01%(67/77),均显著高于对照组的41.56%(32/77)、84.42%(65/77)、66.23%(51/77);并且观察组2年内复发及出现出现神经功能障碍的比例显著低于对照组,而在生理功能、躯体疼痛、情感职能以及精神健康等4个指标的评分均显著高于对照组,上述差异均有统计学意义(均P〈0.05)。结论:采用神经外科锁孔术治疗颅内肿瘤不仅手术时间短、出血量少,而且治疗的有效比例更大。术后出现复发及神经损伤的比例也更低,明显提高病患的生活质量。相对于骨瓣开颅显微术,神经外科锁孔术更适合在临床上推广应用。 Objective: To explore efficacy of the bone flap craniotomy and keyhole surgery of neunosurgery with patients. Method: From January 2013 to April 2015 in our hospital,154 cases of patients with intracranial tumor were randomly divided into observation group and control group by digital method,with 77 cases in each group. The observation group was using keyhole surgery of Neurosurgery,the control group was given with craniotomy surgical microscope. The two groups were compared in the operation,and the quality of life was evaluated. Result: The operation time and blood loss in the observation group were significantly lower than those in the control group. The cure rate and treatment efficiency and the survival rate 2 yrar in the observationgroup were respectively 63.64%( 49 / 77),96. 10%( 74 / 77) and 87. 01%( 67 / 77),were significantly higher than those in the control group 41.56%( 32 / 77),84.42%( 65 / 77) and 66.23%( 51 / 77). The proportion of neural dysfunction and recurrence within 2 years in observation group was significantly lower than that of the control group,and the scores of the four indexes of physiological function,body pain,role emotional and mental health scores were significantly higher than those of control group. The difference was statistically significant( P〈0. 05). Conclusion: The keyhole surgery used in the treatment of intracranial tumors has not only shorter operative time,less bleeding and but also has greater effective ratio. The proportion of recurrent and nerve injury after operation is lower,and the quality of life of patients is significantly improved. Compared with bone flap craniotomy,keyhole surgery of neurosurgery is more suitable for in the clinical application.
