Objective： To explore efficacy of the bone flap craniotomy and keyhole surgery of neunosurgery with patients. Method： From January 2013 to April 2015 in our hospital,154 cases of patients with intracranial tumor were randomly divided into observation group and control group by digital method,with 77 cases in each group. The observation group was using keyhole surgery of Neurosurgery,the control group was given with craniotomy surgical microscope. The two groups were compared in the operation,and the quality of life was evaluated. Result： The operation time and blood loss in the observation group were significantly lower than those in the control group. The cure rate and treatment efficiency and the survival rate 2 yrar in the observationgroup were respectively 63.64%（ 49 / 77）,96. 10%（ 74 / 77） and 87. 01%（ 67 / 77）,were significantly higher than those in the control group 41.56%（ 32 / 77）,84.42%（ 65 / 77） and 66.23%（ 51 / 77）. The proportion of neural dysfunction and recurrence within 2 years in observation group was significantly lower than that of the control group,and the scores of the four indexes of physiological function,body pain,role emotional and mental health scores were significantly higher than those of control group. The difference was statistically significant（ P〈0. 05）. Conclusion： The keyhole surgery used in the treatment of intracranial tumors has not only shorter operative time,less bleeding and but also has greater effective ratio. The proportion of recurrent and nerve injury after operation is lower,and the quality of life of patients is significantly improved. Compared with bone flap craniotomy,keyhole surgery of neurosurgery is more suitable for in the clinical application.
