小儿围体外循环期血浆肾上腺髓质素及细胞因子的变化 预览

摘要 目的观察小儿心肺转流(CPB)前后血浆肾上腺髓质素(ADM)及炎性因子的变化。方法非紫绀型先心病年龄1~4岁,分别于麻醉诱导前(T0)、CPB即刻(T1)、主动脉开放后5min(T2)、CPB后6h(T3)和CPB后24h(T4)抽取桡动脉血测定血浆ADM、白细胞介素-6(IL-6)、白细胞介素-10(IL-10)及肿瘤坏死因子α(TNF-α),记录CPB时间、主动脉阻断时间、术后呼吸机辅助时间、心脏复跳情况。结果 IL-6、IL-10浓度在T2、T3时显著升高(P〈0.05)。ADM在T2、T3、T4时明显升高(P〈0.05)。结论 CPB可导致血浆ADM浓度明显增高,且与IL-6、IL-10、TNF-α浓度升高在时程上有一致性,但ADM的升高更为显著。 Objective To evaluate the effects of cardiopulmonary bypass (CPB) on plasma adrenomedullin (ADM) and cytokines in children undergoing open heart surgenry. Methods Twenty children aged 1 to 4 year, were enrolled in the study, Blood samples were taken before operation ( T0 ), aortic unclamping ( T 1), at 5 min during CPB ( T 2 ) and 6h ( T3 ) and ( T4 ) after CPB for detecting plasma TNF-α ,IL-6 ,IL-10, and ADM concentartions. The time for aorta clamping and CPB were recorded. Resuits Plasma ADM eoncentartion was significantly in creased at T2-T4 as compared with those at T0 (P 〈 0. 05 ). IL-6 ,IL-10, TNF-α Level at T2 ,T3 where significantly higher than those at T0, T1 (P 〈 0. 05 ), There was no significant correlations between ADM levels and aorta clamping or CPB time. Condusion CPB can cause the increase of ADM and TNF-α, IL-6, IL-10. The changes of ADM levels may be related to the CPB-indueed in creases of TNF-α,IL-6 ,IL-10 levels.
