早期肠内营养对SAP大鼠模型营养学的影响及机制研究 预览 被引量:7

The effect and mechanism of early enteral nutrition on the nutrition of rats with severe acute pancreatitis

摘要 目的探讨早期肠内营养对重症急性胰腺炎(severe acute pancreatitis,SAP)大鼠模型营养学的影响及机制研究。方法将60只SD大鼠建立SAP模型,随机分为两组,对照组(n=15),采用肠外营养(PN)治疗;实验组又分为3组(n:15),采用不同时机启动肠内营养(EN)治疗。比较治疗后肿瘤坏死因子(TNF—α)、血淀粉酶、白细胞介素10(IL-10)等病情指标,以及血清白蛋白(ALB)、转铁蛋白(TRF)、总蛋白(TP)等营养学指标,取肠道标本观察黏膜厚度及绒毛密度情况。结果治疗后实验组TNF-α、血淀粉酶、IL-10水平较对照组有显著性差异(P〈0.05),ALB、TRF、TP水平较高(P〈0.05):病理检查显示:实验组肠黏膜厚度和绒毛密度情况均较对照组好。结论早期肠内营养能更好地控制病情,可以改善SAP大鼠模型营养学状况,EN启动越早恢复越快。 Objective To explore the effect and mechanism of early enteral nutrition on the nutrition of rats with severe acute pancreatitis. Methods 60 rats were builded SAP model, divided into two groups, the control group(15 cases) received routine treatment by parenteral nutrition (PN), the experimental group were randomly divided into 3 groups (15 cases), received enteral nutrition(EN) in different time,to compare TNF-α,blood amylase,IL-10,ALB,TRF,and TP. Results Compared with before treatment and the control group, the TNF-α, blood amylase and IL-10 of experimental group were lower (P〈0.05), the level of ALB,TRF,TP were higher. Compared with the control group,the height of the villi and the thickness of mucosa were significantly better in the experimental group. Conclusion The enteral nutrition can control illness better, improve nutrition of the rats with severe acute pancreatitis and start earlier that make patients get well faster.
